FAMILIARIZE yourself with ALL sections of the ReadyVet™ Your Pet's Medical History, Emergency Response Book. We suggest that each of your pet's have it's own copy of ReadyVet™.
GATHER the following information for you pet (if applicable).
Medical Records: (Include ALL perinent medical records. Obtained from your verterinarians, veterinary hospitals, laboratories, etc.) Daily Living Needs, Implants and Pacemaker, Disabilities, Vaccinations, Surgeries & Procedures, Tests, Treatments, Medical Appointments and Emergency Room Visits.
Medication Records: Prescription Medication, Over-the-Counter Medications, Vitamins and Suppliments.
Allergy Records: Critical Allergies, Allergist and Treatment.
Medical Insurance Records: ALL Medical Insurance Policies.
Breeding Records: Complete Breeding Records.
Diet: Regular and Special Diet Information.
Governmental & National Contacts: Specialized Pet Agencies.
Legal Records: DNR and Advanced Directive, Organ Donor Information.
Maps: Freeway and Residential Maps of Your Area.
Personal Records: Emergency Medical & Veterinarian Contacts, Pet Service Contacts, Family & Family Contacts, Neighborhood Watch & Neighbor Contacts, Pet Photo.
